The stats of the slayer are too low for the DCs to work properly on tactician so none of the conditions that let you do massive damage actually fire.
You have a multi attack with is like 6 1d4 attacks but it automatically crits if the enemy is dazed AND does like 7 extra damage per hit if the target is prone (or maybe it was bleeding)
Theoretically you should do decent damage, instead you do less damage than a standard attack because half of what I just mentioned doesn't even work as written.
The form is rather simple, one action, one bonus action
