I believe this one is real. But the actual “Ratman” is just a random hobo who sometimes spawns in that area of the map.
Another one related to GTA IV I know of is that if you shoot at the heart in the Statue of Liberty, it causes your game to glitch out and become more unstable, leading to your game eventually becoming unplayablely buggy.

Actually true was the ghost car, a full health car with full cosmetic damage that spawns at the top of a hill in the wood and rolls down it.
Not sure if true was "The murder hillbillly," apparently the guys driving tractors in fields were not programmed to stop for people in the way, so they would "Chase you down and run you over"
IIRC there's also a couple of misplaced spawn points where NPCs would spawn on top of lamp posts and then instantly fall to their deaths.
Some of the stuff people claim about Sad Satan being supernatural, but the real details about the game is really fucked up. Another urban legend is Luigi being a hidden unlockable character in Super Mario 64.
The game is pretty much dead from what I've heard, which is a shame but just as well since it seemed to be oozing with feature creep. Also Chapter 6 of Replay came out in March, what the fuck
