What is medical magic like in your worlds? Common? Rare?
Is it possible for magic to cure cancer? Leprosy? The common cold? Dysentry? Polio? Schizophrenia?
What are the hospitals like in your worlds?
Is it easy to get magic cures for diseases? Or is that impossible outside large cities?

    Healing magic is extremely rare and tends to only work on flesh wounds and such. It's similar to forcing growth or roughly transmuting flesh back together.

    Diseases and poisons are mostly unaffected by healing magic and much be treated via alchemy and potions.

    I mostly set it up this way to prevent diseases and poisons from being a meaningless joke as soon as there's a Cleric or Paladin equivalent in the party, because I hated how trivial those things were in D&D.

    Magic in general is rare and split into two types; Faith based or Imbibed.
    Faith based for Paladins, Clerics, and Monks that are derived from faith in a cause, ideal, or deity.
    Imbibed is for all others in which they must procure a special elixir that allows them to tap into the arcane realm, only have to take it once.
    Wizards often get it from a Master Wizard or Witch that takes them under their wing, Warlocks from their Patron, etc.
    There are no spell slots but each time a spell is cast, one has to roll for spellcasting ability, the difficulty of the check varies by level of spell. Successful rolls casts the spell as normal, Failed rolls do nothing but waste an action. Crit success enhances the spell (equivalent to casting at a higher level), Crit failure results in either a level of Lost Faith or Corruption depending on the type of magic. After four levels of LF, the characters completely lose faith in whatever it was that let them use magic. After four levels of Corruption, the character is reduced to a mutated abomination. Though there are ways to lower the level of Corruption or regain Faith before the max is reached. All of which are either expensive or time consuming.

    The high risk involved makes it a gamble for PC's and npcs and they have to really think through whether or not to cast a spell to cure someone of something or heal wounds. Most rely on Alchemists/Apothecaries to make Tonics and such that take longer to cure lesser diseases with bedrest and routinely taking them. Surgeons often deal with physical injuries as best they can with the tools and know how avaliable.

    The rarity and risk involved makes medical magic especially seem like miracle work to most in the world. Basically, someone like a Cleric walking into a village would almost necessitate a kind of celebration for such a godsend.

    I once read an AtLA fanfic (which I unfortunately cannot remember the name of) in which Fire and Earth-based methods of healing were discovered in addition to the canonical Water-based healing, so I decided that each of the four elements would be able to heal. I was thinking that Air healing would focus on the respiratory system, do you think that this works or does it need expansion?

      Some branches of Eastern medicine focus on the circulation of chi, which is heavily (though not exclusively) associated with breath. It makes sense that the different nations would have their own mundane and Bending-based takes on this, and I like the idea of Air having special ties to the respiratory system. What are you doing for Fire and Earth? Sweating out impurities, pinpoint cauterization of tumors, removing heavy metals, manipulating mineral deposits (bone, kidney stones)?

        Earth healing would focus on the mineral manipulation point that you mentioned, especially the bones, and Fire healing would do things like manipulating the body heat of the patient to kill infections, along with some techniques involving the nervous system, does this make sense to you? And do you have any more thoughts on Air or Water healing? Because at least for the latter I should probably try to make it more distinct that AtLA healing, but even emphasizing the blood and circulatory system aspects just brings bloodbending to mind.

          Oh! I misunderstood and thought this was for an Avatar RP.
          If you're making your own thing, then I wouldn't worry too much about Water. For one thing, bloodbending was very difficult and rare in Avatar, while your water-based healing magic can be relatively simple and common. This magic might treat burns (suffusing cells with water), cleanse the bloodstream and urinary tract (including poisons), or keep blood from leaking out. Air healing can cleanse the respiratory system, and even leech poisons from the body, though less effectively than Water. Air might also be the best for circulating chi (if something like that exists in your setting), with a range of health or magical benefits, though the other elements can probably do minor chi cycling themselves. If you don't have chi, then Air might still "energize" patients in the same way that oxygen therapy does: increased endurance, alertness, concentration.

          Chi aside, I'm basing these ideas off real-world biology. But you also have the option to say that people work differently in your setting, or that the magic just heals through mystical resonance rather than interacting with bodily functions.

            Well, I 'was' considering that, but I wanted to see what it would be like as something more standalone first, find out if it was something people would be interested in if it wasn't from Avatar you know? So ideas for either option work.

      Air could focus on maladies of the mind / emotions by guiding meditation towards stillness with a kind of "calming the storms" motif.
      Fire could focus on purification / sterilization and cauterization of course.

    Magic healing is very focused and limited because using magic on another person is treated like being invaded by foreign bodies. Even if the person has never heard of the magic their bodies will naturally reject it so doing things like rendering a person on concious via sedatives and such is necessary to lessen the effects.

    Even then, it's mostly used to check on the status of health and even that isnt always accurate and requires actual medical knowledge to assess the information you are gaining.

    You can boost someone's natural healing if they don't actively resist you but this can also become addictive to the point where you can no longer heal on your own without assistance and finally to the point where you can't at all and your body sufferes a rapid deterioration and cell death.

    "Healing Magic" only truly work on spiritual beings, for which restoring their shape & coherency is semi-easy.
    On corporeal beings the effect last only as long as the caster feed it with power, as it's closer to a temporary & very superficial rewriting of reality - borderline on being just an illusion.
    Good enough to keep someone fighting or bring him safely to the emergency room, but at some point actual (non-magic) medicine will have to be done. It's pretty much useless against diseases, unless you just want to relieve the pain and/or crippling effects for a while.

    Healing magic has its uses, but is inherently impractical for most applications.
    >In war
    A purely arcane healer in a medic position on a battlefield would burn through all their energy in a few minutes and only be able to save a handful of soldiers, a drop in the bucket of a single battle's casualties. Due to this, and the fact that no one has the resources to field one healer for every four or five foot soldiers in a massed-levy situation, the medical tents are staffed primarily by entirely mundane sawbones. A few magic healers (who are preferably also trained in mundane medicine) are kept on-hand for emergencies to ensure the survival of higher-ranking officers and nobles if they're injured
    >Doctors
    Same difference basically, a magic physician either learns how to treat injuries and illness without magic or he's only saving the first few people who knock on his door each morning. If a smaller village is lucky enough to have a magic healer, they might be able to get by using their magic for the sum total of injuries and sickness that arise in the course of a typical day, but that will change rapidly if there's an outbreak of plague. Additionally, because you're killing the sickness with magic, the body's immune system doesn't learn how to properly fight it and a disease like smallpox that would ordinarily only have to be fought off once to acquire immunity will be just as deadly if the same person is exposed to it later in life.
    >Adventurers
    This is the use-case where healing magic works the best. A healer traveling with adventurers can focus their efforts on only a handful of people, and their companions are often hardy enough to walk off minor scrapes and conserve the magic for when they truly need it.

    You go to see the shaman. He doses you with lsd and plays the drums for you while you hallucinate a spirit vision. This will make you feel much better it may or may not be of medical use.

    In my setting all magic belongs to either Law or Chaos, and I'm trying to come up with spell ideas for both sides, including healing. For Law healing I was thinking that they'd essentially do a "factory reset" for the damaged part, restoring it based on what the body 'knows' it's supposed to be like, at the cost of not being able to fix things like congenital disorders, due to them being in said "blueprint", but that leaves me wondering what would work for Chaos healing besides causing the body to mutate until it reaches something that is simply "close enough", got any ideas please?

      You can heal people, but speeding up that healing process shortens their lifespan.

        That's an interesting idea, and I might actually use it, but it still doesn't help me on the Chaos healing side of things, besides the possibility that Chaos healing extends lifespans if Law healing shortens it.

          Chaos healing introduces the risk of healing you 'wrong'. Its best used for minor injuries, which generally heal without consequence, but bigger injuries like large scale burns or missing limbs tend to grow back something that doesn't look like the rest of you. It could be a weird color, or be covered in boils or scales, or something weirder. You WILL be fixed up, but you'll look weird and the more you do it the more the weird changes stack until you are just a mismatched monster man.

    Standard fantasy fair, healing magic and healing potions.
    Magic can cure everything from cancer to the common cold and muscle soreness.
    Hospitals are run by the church, but even small turns have a healer.

    The setting is medieval but going through unequivalent of the 12th century Renaissance. This is making the god of undeath and stagnation very upset. He will release a magic resistant plague, the equivalent of the black death, as a major plot point.

    Something that actually worked apparently. In the medieval times and noways. Well for now.

    In March 2015 it was reported that Bald's eyesalve recipe – garlic, leeks, wine, and the bile from a cow's stomach left in a brass bowl for nine days – was tested in vitro and found to be as effective against methicillin-resistant Staphylococcus aureus (MRSA) as vancomycin, the antibiotic used for MRSA. The ingredients separately were not effective, but the combination was.[10][11]

    It has been suggested that a lot can be learned from medieval medicine because wounds must have been ubiquitous in agrarian societies: "If you cut yourself with a scythe, it was highly likely that you'd get an infection." In particular, leeches and maggots are returning to medical use in the 21st century.[12][medical citation needed]

    How to make it:
    Also mentioned is a special eye ointment that consists of ox bile, wine, garlic and onions and which English scientists have put to the test. To make "Bald's Eye Salve", the onions and garlic cloves were first crushed with a mortar and then mixed with the ox bile and wine. This mixture was then left to stand for nine days and finally filtered through a cloth and poured into a horn. According to Bald's Leechbook, the tincture should then be applied to the eye at night using a feather.

    Does a cleric/priest/mage casting healing spells needs to know anything about the body in order for magic to work?
    If I want to use magic to knit this broken bone back together, do I need to have a mental image of a healthy bone?
    Or do you just go to your local magic quack, have him say "Bippity boppity boo" and now your broken arm is fully mended, back to the farm you go.
    Is there a genetic component? What if I was born without arms? Can magic "restore" my arms?
    For a cleric, would greater faith lead to more powerful healing?

      Clerics don't.
      Mages do.
      Which is why 99% of all healers are clerics. It's just so much more work if you don't have divine influence manage the "how things should be" for you.
      On the other hand, the few mages who do bother can seamlessly go into biomancy.

          They get divine cheat, they also get no control over the result.
          If their god decide you deserved that leg injury, you might stay partially limping forever.
          Meanwhile a competent mage might make it humanly better than it was before the injury.... or get you killed in a few weeks because he pushed away one of those pesky artery and its now flushing your bone marrow away or another "oopsie" in the same vein.

    Healing requires prolonged meditation. The best you can do through mundane medical skills is stop someone from bleeding out to death.
    Supernatural means of healing are slightly more powerful, but they can't stack, and are very very rare, limited to sages and alchemists.

        A doctor has a better chance of patching it up, but yes. The only reason you'd travel to seek a doctor is if you're hit with an illness, or poison.

    In my setting most magic is medicine. They believe in the 4 humors but take them out of ill people and mix them to get a desired result.
    So you take some from a leper, mix it with spirit, belladonas and warm it up. Now you habe a pain killer that allows you to push your pain resistance.

    You want to be more ferocious in battle, get some rabies! Mix it with water, belladona and wormroot and you get an injection that will make you stronger.

    The only issue is that repeated use of these things will inevitable degrade your body and bring you to a horrible death. But who cares when you can create rabies + aderal?

    Actual healing magic is powerful, but pretty damn limited in both resource and application. You might fix up half a dozen with common traumas in a jiffy, but then you're out if juice and can't even heal a scratch. Diseases and poison need specialized spells or rituals, which complicates the matter. Not to mention that healing drains the target's natural vitality. If strained by fixing extensive injuries their body will simply exhaust its natural regenerative properties until rested. If they still ain't fixed up then pray someone's around for vitality transference, because that person's immune system just shot down and their blood can't clot for hours.

    On the other hand, more classical medieval forms of healing have been made actually viable with supernatural support. A doctor does not need to be a mage or cleric to mix together potions, filter "tainted" blood through troll liver, balance the four humors with herb concoctions, or trigger the metamorphosis of stored changeling bodies to make perfect transplants. In fact there's a medical organization of bloodsuckers that specializes in making bloodletting actually work + holds the secret of converting blood types, storing massive amounts of universal donor blood.

    It can cure most illnesses or damages but often has trouble with mental illnesses unless they are caused by a brain injury.
