Secret cults on means that you have to keep your chaplain hunting apostates non-stop or a large part of your realm will pop up as infidels. Though it's easy to figure how widespread cults are in your realm by just checking for sympathy traits.

Half my realm still converts to heathen religions constantly.
But because I don't have the DLC, I can't combat this, and the (Christian) AI can't do shit about it either.

Whenever the religion of the realm I'm currently playing as gets so powerful that others can't do anything anymore I try to undermine it with heresies or converting to other major religion. I like this because it keeps religious conflicts balanced and interesting, it's better than having crusades conquer fricking Iceland.
I turn off secret religious cults but the satanists can stay, even though I basically never join them since then you feel obligated to become an emperor tier antichrist and instantly annex the world.
The real question is about supernatural events. And if you have cults on, you must have supernatural stuff off. Otherwise, the game goes into meme overdrive.

On, you can generally supress them easily and it gives you excuses to kill/imprison vassals. The only thing to watch out for is that if you have a religious title (e.g. Caliph) conversions from the religious sects can frick up your succession. It also sucks that low RA doesn't do anything to the cults. I've had a Shia campaign where I conquered pretty much the entire Muslim world and there was maybe a dozen Sunni provinces left on the map but almost my entire family was secretly Sunni for generations and kept offering me to read the Sunni Quran for hundreds of years. Compared to this the satanists are nothing.
