AO rated games can't be sold in retail stores on on their digital storefronts (outside of Steam but Steam still censors many things), you can only special order from niche websites like Mangagamer or something

the problem isn't really with publishing, it's because it's very hard to make realistic sexy animations/graphics with humans.

Sexualising women is only ok when the woman consents, and morons in the west think fictional women also need to consent to be sexualized because they represent real women (ironically you don't need them to consent to violence or drugs). and since men are often creating said fictional women then it is done without consent. And even if a woman does create said character it still doesn't count because she probably has a male boss.
