If they magically had someone with the knowledge and understanding to make something no other doctor could then fine, suspension of disbelief whatever activated. Society was deteriorating in the beginning of the game not because of the mass hysteria of a group of people, but because a soldier could rationalize killing an unarmed man and child off of the fear that they might be dangerous. In the end of the game society was still deteriorating because a scientist could rationalize killing a child off of the possibility that it might produce a cure.
They are in very good shape and waste their privilege immensely. They've basically restored late 20th century rural living conditions and go on massive killing sprees against each other. It's actually a lot like third world countries.

You still completely disregarded half of what I said. It's been 20 years. The hospital room was grungy and had flood lights. If they magically had someone with the knowledge and understanding to make something no other doctor could then fine, suspension of disbelief whatever activated. Throughout the entire game, Joel and ellie encounter people who betray them, cannibals who would've raped and killed ellie, joel encountered a retard soldier who "followed orders" and murdered his daughter, and the fireflies themselves were fucking delusional terrorists and were about to kill a girl without even asking her if it was okay to do so (the argument that she wanted to die is irrelevant here because they don't know that). Why should Joel sacrifice his one chance at happiness to save a world that has been cruel and heartless to him? They simply don't deserve to be saved. Your fauxutopian standing falls apart under basic scrutiny because all of society relys upon the close knit personal relationship between people as a building block. Expecting a person, any person, every person, to chose the lives of everyone else over the life of one person he cares about is inherently at odds with utopian society because it requires a break at the foundational level.
To put it in simple terms, a society that deems it necessary for a father to sacrifice his daughter for the good of the all, is not a society worth that sacrifice at all.

As another anon pointed out, there is a massive difference between asking someone to sacrifice themselves (or in this case sacrifice their daughter) and forcing that sacrifice upon them. What the fireflies were going to do is quite literally murder, regardless of the potential reward for it. So when looking at their actions, the terrorists hide behind a hypocritical ideology to achieve their goals, while Joel has as an unrepentant attitude of "fuck you, got mine" which in this case is the right thing to do because it's a decision based on love and value. His actions are not only more relatable to the average person, but more morally correct because they stand upon themselves without deception as a crutch. If ellie had been conscious and decided to sacrifice herself, and THEN joel stopped it, he would be in the wrong, as he would have been taking away her agency. But that's not the case.
