such a kino game, if only the base game had more factions, regions, political systems like the pope or senate in rtw and more historical accuracy.

CK3/CK2 are the best for this.
Do I need all the DLCs?
Arguably - yes if you want the best experience.
Especially for CK2 which has specific Byzantine/Rome DLC which is a must have for that kind of playthrough.
It's stupidly overpriced trying to get all DLC since paradox always breaks their games down into shitload of expansions to make up for a full game.
So just pirate it.

I recommend Medieval 2 with Stainless Steel with the late campaign start, it starts you right after the crusaders have captured Constantinople.
