Last character I made was in Aliens Fireteam Elite and it was some black guy with bleached hair. It felt appropriate for the setting.

Depends on the game and the class. I just make a dude who I feel would fit the setting and my idea of the character. Sometimes I end up making vaguely self-insterty looking characters, sometimes they look nothing like me.
Either an idealized self insert or my ultimate waifu. It depends on how good the character creator is. If both fail I just go with "decent looking guy".

Self insert first time, well to a point it feels right for the game.
Second time a complete different like female or wacky character, after that I make unusual builds with characters that fit the build.

I always make someone fat as frick
Hit random until I get something vaguely human looking. Remove degeneracy like tattoos, piercings, neon hair or makeup.
It's been so long since I cared about character creation, but I typically just pick the skinniest/slimmest guy that's agile and fast and cool.
And then try to model my face (always fail) in which case I'll make a femboy if I can, and sometimes games will have all the male character only be allowed to be buff or chunky, so I'm then forced to pick a female character which is awkward, but atleast her body type is closer to mine. I then proceed to try and make her as boyish as possible.

In any other game with a character creator I just make sexy women.

I either make a sexy girl or a monster, however 90% of the time it's a girl because most games don't let me play as monstrous races
First playthrough I make me, subsequent playthroughs it's a random toss-up between some character I made up, a character from some other work, some eye candy, or the ugliest sack of shit the character creator will let me make.
Usually I just play around with it a bit until I find something I feel like sticking to. If there's classes involved I'll usually try to make something that fits them.

When I was young I made my ideal. Nowadays I just try to make something hot and cute. I don't even fap to it, I just like to see something that makes me feel.

I make what feels setting appropriate and change as few sliders as I can while still departing from the default character look. Often I do this with hairstyles. I pick a preset, change the hair, maybe add a scar or tattoo, and run with that.

I try make an old male chad for first playthrough, and if I think the game has skimpy sexy clothing for females my ideal waifu for second playthrough

I pick a character I think who would be interesting to see go through the game and roleplay thay character to the best of my ability. Like playing a ttrpg.
Mage elf, I always go for the mage elf looks. If they don't have an elf race in their game, I would role the equivalent of that aka thin smart boy.

I design a character that I feel either fits the supposed world or the type of personality I want to give it should the game allow me a choice there. I can immerse myself in a game but self-inserting to the point where you believe you're that character is weird to me.
For example when I played FFXIV I made a female at the start because I wanted to heal, didn't think too hard about it at the time so thought a girl would fit the whole white mage shit. Once I got tired of that and wanted to focus on tanking or DPS I changed to a guy.
I try to make a character that matches the tone of the game. Serious character for serious games, goofy character for goofy games, sexy character for sexy games.
In multiplayer games I try to go for either over-the-top goofy or subtly cool so I guess that’s my true revealed preference.
