Obviously the top one. The point of dual wielding is the loss of accuracy, but twice the fire power, and it is more fun to hold down two mouse buttons or spam them both to fire tons of bullets.
Depends on if you want to intentionally lock out whatever mechanic would normally be on M2 (aiming I guess would be the most common for an FPS) in exchange for having two guns at once firing independently. I prefer this because it's more interesting, but I also don't really have a problem with the alternative.
I prefer bottom. CoJ Gunslinger works like that. It also allows for ADS, which is necessary because of how loose hip-fire is in that game for every gun
Take the moronic payday 2 route where clicking once fires both guns, effectively making the point of dual wielding null. Now you just have a gun which does twice as much damage.
M1 with alternation between each weapon and slightly higher firerate than just having one weapon.

depends on if you use same 2 guns or not
pic 1 worked great for bl2, otherwise pic 2 makes the most sense
realistically, there's no reason you need to fire each gun separately
