I would keep the entire series unchanged and continue to print slop like Shelter and 76, because the only worth this franchise has ever had is as a money printing machine to fund TES.

I'm not gonna lie to myself that Fallout only exploded in popularity when Bethesda turned it into oblivion with guns, despite being aware how they assraped the worldbuilding.
If it was released today, it would be like Fallout 1 but in 3D and with more intuitive controls
I would change the first random encounters in Fallout 2 to ramp up in amount of enemies slower because good fricking lord
1. Remove speech checks from all final bosses
2. Rework speech in general
3. Combine 3D world cells with 2D overworld travel so for example you'd have a smaller 3D Vegas area map and you would be able to go to 3D boneyard using fallout 1/2 style travel if you get a car.
4. Get rid of Sierra Madre vending machines, cold fusion from the tv show and all other kinds of infinite resource bullshit
5. GET RID OF SYNTHS. All robotics and cybernetics are visibly distinct, no more of this lifelike bladerunner malarkey.

I agree about the Sierra Madre machines,I just can’t accept they are literally matter replicators or rearrangers I choose to believe there’s some kind of trick to them. Like maybe there’s a limited supply of slop stored in them that that they turn into junk food maybe
I also agree about synths and the institute as a whole- in Fallout 4 they look considerably more advanced than the Enclave,

VATS in fallout 3 was supposed to be a turn based system but I think bethesda never managed to make it actually work so they made it just an "aim bot" system.
Keep plot of 1, 2 and New Vegas mostly unchanged. Buff every stat in SPECIAL system to be on par of dexterity in Fallout 1. STR gives huge percentage damage bonus to melee weapons. With 10 str you treat heavy armor as light armor and can use two handed weapons with one arm. High charisma unlocks overpowered companions like Joshua Graham. Int increases exp gain and lvl cap. High perception operates like Auspex in VTMB.
At this point I'd settle for the TV show being non-canon or an alternate universe. Anyone who says the show is lore friendly or a good representation of the setting has never played 1 or 2 and thinks the entire series has always been like 4.
