Ultima is probably the most isekai of all video games as the protagonist (Avatar) enters the world of Britannia from the real world in the beginning of games and often returns back to Earth between games.
I wish the Twelve Kingdoms was more popular. There's two Japanese only games that look like they probably suck, but it's all there is. Never really cared for any other isekai

These are all action games though, not RPGs.
