Just incompetent morons on camera and in script writing. Perhaps the only people that are any good at their jobs there are the people in production(cameramen, audio, effects, etc).

There are other aspects of it that we do not see, if you consider the man's attention divided between being the "vision officer" and being an on camera personality, there's one place where inattention carries less of a risk. i.e. show up, do the shoot, walk out. Everyone else can handle everything else. Even in the combo copper block he's the one to realize that they were using the wrong gpu. But since the shoot was already going, stopping to go get the correct one may not have been in his schedule and may not have been in his best interest considering all the "data" that had already been gathered and collected into a script for him to read.
