That dialogue is in the game. It's much more clear when you play as Nora, where she respond sarcastically about getting pregnant again when Nate suggests going to the park.

New Vegas can get away with having a backstory because it's one of those "fresh start" RPGs, like Planescape Torment. If the protagonist goes through an event that causes them to basically become an entirely different person (like fricking dying and coming back to life, or having a piece of your brain shot out), then it's okay because after that, the character's motivations and values are up to you.
