In her game, she's considered hot. In game, Rihoko and Kaoru are considered the "normal" or "plain" ones. And of course it doesn't look that way to us outside observers. It's very hard to draw plain or ugly in anime, aside from giving a girl a ton of wrinkles, drawing their face unbalanced or giving them a deformity.
It's not hard, it just doesn't sell to a wide audience.
No, I think it's legit hard. Draw a "common" anime girl, like some one shot girl you see for 2 seconds in the background of a random anime, and they still end up looking not plain. They stand out. You actually have to work hard to make an anime girl look bad. Usually by giving them some flaw like uneven hair or a big nose or something. And even then, someone will still fap to it.
There's a difference between looking approachable and looking beautiful anon. Nobody falls in love with mob characters.
