They originally designed it to be a 2.5D game like the older ones, with the world map picking the segments but said it was working fine in 3D
It's why the open world is so barren outside of the few designated 'here's shit to do' spots
The difference is the empty space in 1 and 2 is abstracted without random encounters and traversed quickly on foot and even faster by car. New Vegas is just static enemies and empty shacks

in new vegas it reflects how well you can strafe backwards and click on heads
The writing isn't good either it just has a lot of pre-baked options.
