I have made up several characters that I always recreate in whatever games I play with character creators, so I just use their full names when I make them. They're usually relatively obscure combinations that are almost always free. If the name is taken - I go the emergency route and use a fancy pretentious nickname I've come up with for each of them, worked well so far.

Or I just go on a lark with Baron Chadwick Von Hammerkök II, Mister Lister,
name myself after a reference to mein kampf, johnny rebel songs or moonman, and see how long until someone gets it and squeals. then, racial slurs in game chat/mic follow
rinse and repeat
I went to school where there were three brothers named Hubert, Quebert, and Eggbert.
So if I ever can't come up with a name I just go with one of those.
