Lag/bug.
Everything else, there is at least a justifiable cause, but lag and bugs are not something people can generally control.
In the case of lag, even with near-perfect Internet, ensuring your ISP isn't sending your connection through Africa because it saves them a penny every 5 minutes, AND the host servers are in good working order too, lag can still occasionally just happen and the only thing left at that point is to be jacked directly into the server at its source, but that's just unreasonable to expect as a possibility for every single video game out there.
In the case of bugs, you can blame the developer, but even so, it's just frustrating because most developers don't work around the clock and deploy patches constantly to fix every little thing.
Cheaters are a "Justifiable cause," because someone else decided to make your experience worse. You can immediately blame them for it, which at least somewhat alleviates the frustration, unlike lag or bugs, which are almost entirely out of anyone's control on a moment to moment basis.

This. This is as lame as it gets. Bonus points if the game also has an actual unwinnable fight that you're supposed to lose in gameplay.

Lag in recent EA games, because you can't move or do anything at all while everyone else is moving around perfectly fine and you're just frozen.
