Yeah, it's the whole face, i've noticed. Not just one crease or surface. In regards to that one guy, he was a manic depressive. He'd hit the gym when suicidal and pump until he was too exhausted to even lift a knife. Then he'd pop amphetamines and roid out like a loon.
Had to talk him out of my shared apartment one time. I'm probably lucky he didn't swing. Guy would have killed me but something in him was always too insecure to make that kind of a move. At least, that's what I saw in his face.
Pretty sure there are only a few hundred different "types" of people per race.

Dude I talked to in college was also self admittedly bipolar and I was the only one to show up to his birthday party, early I like to get drunk before the sun goes down. Didn't look at all like that but had features that were on the frumpy side of slightly attractive, homosexual erectus looking. 6/10 and a personality to match. I just wanted frens or something, I was 28 and a day student.
So the third time I get drunk with him and tell him I'm gay, no reaction the first two times, he goes on a spiel about not mentioning that at the IT job he got that he could hook me up with. So I got trashed at his 23rd birthday, pulled in 2 ringers to school all his lame friends at Mario Kart and drinking, and made an absolute ass of myself the whole time. Sweet college memories. I'll spare you the rest of those 2 years.

The store stopped selling them but you can find the list of resellers that charge more if you look in the Nintendo store.
