Saving just Cybil nets you a pretty shitty ending. You gotta do Dr. Kaufmann's "side quest" to get a good ending, which involves saving him from a monster at Annie's Bar in the Resort District and then follow a breadcrumb trail of clues that'll lead you to his motel room and to his motorcycle where he's stashing more of the red goo.
you have to use the liquid on her in the second stage of the fight, it wont work in the first stage

I got it first try without attacking her. I always read that fight stages are a thing but i never bothered with that. Just approached and used it in inventory. Maybe she has to be in a second attack cycle or the range is finnicky or something.
When you get to the hospital before it goes into the Otherworld, grab one of the plastic bottles and use it to scoop up some of the red goo on the floor in the director's office. When you fight Cybil and you lower her health enough that she drops her gun, get close to her and use the red goo on her.
I figured this out on my first playthrough intuitively. Didn't find the good ending trigger though so got Bad+. Also i missed the shotgun in the school.

Doctor's second appearance is a little side plot in a section of buildings you're never directly told to visit.
