Being a homo. She was skinny and long brown hair. Her getting fat and having danger hair was signs of mental illness. Thankfully, giving her the D was the correct option.

Is it true Japan hated this thing (assuming they got a translation somehow) because of how offensive and crude it was on the topic of cripples?

The term "Katawa" is a slur against disabled people. Rather than "disabled/disability" a closer translation would be "cripple/crippled," but it's arguably even harsher than that. "Katawa" literally means "one-wheeled," the implication being that the person is "broken" and "useless," a burden on others. It is not a term you would ever use in conversation.
Of course, the name of the game comes from RAITA, who came up with the original concept that the game is based on. This was a bonus page in a doujin sold at Comiket, and aside from Comiket's disregard of all social norms and decency, the title was just another way of punctuating the absurdity of the idea. It was just a funny concept that RAITA never intended to develop further.

Despite all of that, the game as it was written by the Four Leaf Studios team is nothing but respectful and humanizing toward disability. The game is told from the perspective of a newly disabled young man, and critically examines his own resentment and prejudice about disability as he adjusts to his new living situation and becomes familiar with other students who all have their own different perspectives on disability. There's nothing to be offended about in the game's script, unless you can't stomach a few self-hating "cripple" remarks or blind jokes from blind characters.

Oh, to more directly answer the question, 4LS did get their own booth at Comiket to sell artbooks and other merchandise for a few years, I assume it's popular enough for that to have been worth their time and expenses.
RAITA did acknowledge and approve of the game not long after it was published, and the game has been translated by fans into Japanese and other languages.

I feel really sorry for people who played katawa shoujo and got shizunes route first. They should have just cut her route entirely if the author left at last minute, rather than try and salvage it.
