Japanese cultural belief is that in any proposed binary (fight/flee, man/woman, strong/weak) that there is a middle path that can be superior to the other options. Okama are said to possess the strength of a man and the grace of a woman, and are considered to be male. The closest comparison in the west would be a transvestite, not a transexual.

The reality of the situation is that the concepts of gender identity and stuff like that in japan don't effectively translate into western culture. This is also the case with countries like China, where lesbian stuff (the only stuff I know about really) don't line up with the butch/femme typical lesbian stuff in the west. It's just a difference in how the cultures handle these things (which are present in pretty much every culture since forever).
