Its all just how you chose to look at these sorts of things and how you want to get what you want in the end. A few years popping out one of your enemy's kids so as to find out how to stop the dark lords plans or the return of the evil god of murder to this world is sadly a small price to pay in the grand scheme of things.

From my experience Ganker is the board that's into pregnancy (there's almost always some thread up about why anime doesn't feature more pregnant women, and there's the whole "brood queen Megumin" meme). /tg/ seems to be more into vore the pregnancy (see literally any thread about dragons or sphinxes, as well as vorecubi and all-devouring vore-e-girl being recurring things across multiple threads going back at least a decade). Pregnancy shows up occasionally but not as much, and a significant amount of time it's specifically a thread about Maiesta RPG, which is an RPG where it's built into the core concept of the game.
There doesn't really need to be one, considering swallow whole is a fairly common monster ability in DnD.
Though now that you mention it, I do wonder how it would work. Like, if you look at Maiesta the pregnancy aspect is mostly a fluff detail except for "size" being a stat with gameplay effect (but you could just rename it to "fatigue" and for most classes it would still make sense), with only the broodmother and to a lesser degree gravimancer really leaning into it as part of the "class fantasy", with most other classes being more of a "standard RPG character archetype except pregnant", with the exception of the one class that is actually based on vore fetish but I've never seen anybody actually play without refluffing it.
If you did the same in a vore RPG you'd end up with classes that would basically be standard RPG class with swallow whole ability tacked on (something like a spellcaster able to recharge spell slots by swallowing enemies or something), or base classes on different variations of vore fetish which I feel probably wouldn't result in enough class variety (which is also why MRPG classes mostly follow standard class archetypes; there's only so many ways you can turn being pregnant into a class feature).
You could mix it up by having classes that specialize in eating enemies, classes that specialize in surviving/doing damage to enemies that eat them, and finally classes with support options for their allies.
I would have all the CON-prioritizing classes act as the eaters. >Barbarian >Fighter >Monks
Then the the other Physical Classes excel at surviving getting swallowed or avoiding getting swallowed in the first place >Ranger >Rogue >Paladin
Finally, the spellcasters can fall into either of the two categories depending on their specific subclass.
I think a fetish ttrpg might benefit from using non-class-based mechanics to allow for greater player choice and more granular character characterization

True. A standard class-based game would just end up as "play as a wizard/fighter/rogue, but you also get swallow whole as class ability". I think MRPG works well enough somewhat ironically because for the most part the pregnancy aspect is part of the lore instead as making every class built around giving birth at the enemy or something (you have the broodmother for that, while every other class does its own thing).
If you did make a vore fetish RPG it'd probably be best to have it function on a point-buy system where you can build your own pred. Like you could put points into physical stats and unarmed fighting to abe able to subdue enemies in melee and eat them, or into abilities that let you incapacitate them in other ways to accomplish the same thing, or there could be an option to make the all-devouring voree-girl by putting all your points into abilities that let you swallow and digest anything but at the cost of the character being small and weak otherwise, requiring either an element of surprise or help of other party members to actually swallow any tough enemies.
...Still, I don't think it would work very well as a game. There's only so many ways you can make "I roll to eat the X" work as interesting and balance encounter design.

I've only played a pregnant character once, in a Maiesta game, and never bothered making art for her. I've thought about it but it wasn't a very long campaign and I didn't find the character very memorable since I just came up with her on the spot when my original idea didn't fit the tone of the campaign.
I did, however, get art for the original character concept, which I might use at some point if I have the time to play in another MRPG game.

Slightly off-topic but I had an idea for a character in pathfinder a while back. A female alchemist with two extra arms that she would keep hidden under her armor most of the time in a way that made her appear pregnant. Never got to use that one. I suppose a male character could pull it off, too and just pretend to be fat instead of pregnant.

From my experience Ganker is the board that's into pregnancy (there's almost always some thread up about why anime doesn't feature more pregnant women, and there's the whole "brood queen Megumin" meme). /tg/ seems to be more into vore the pregnancy (see literally any thread about dragons or sphinxes, as well as vorecubi and all-devouring vore-e-girl being recurring things across multiple threads going back at least a decade). Pregnancy shows up occasionally but not as much, and a significant amount of time it's specifically a thread about Maiesta RPG, which is an RPG where it's built into the core concept of the game.

There doesn't really need to be one, considering swallow whole is a fairly common monster ability in DnD.
Though now that you mention it, I do wonder how it would work. Like, if you look at Maiesta the pregnancy aspect is mostly a fluff detail except for "size" being a stat with gameplay effect (but you could just rename it to "fatigue" and for most classes it would still make sense), with only the broodmother and to a lesser degree gravimancer really leaning into it as part of the "class fantasy", with most other classes being more of a "standard RPG character archetype except pregnant", with the exception of the one class that is actually based on vore fetish but I've never seen anybody actually play without refluffing it.
If you did the same in a vore RPG you'd end up with classes that would basically be standard RPG class with swallow whole ability tacked on (something like a spellcaster able to recharge spell slots by swallowing enemies or something), or base classes on different variations of vore fetish which I feel probably wouldn't result in enough class variety (which is also why MRPG classes mostly follow standard class archetypes; there's only so many ways you can turn being pregnant into a class feature).
You could mix it up by having classes that specialize in eating enemies, classes that specialize in surviving/doing damage to enemies that eat them, and finally classes with support options for their allies.
I would have all the CON-prioritizing classes act as the eaters.

Then the the other Physical Classes excel at surviving getting swallowed or avoiding getting swallowed in the first place

True. A standard class-based game would just end up as "play as a wizard/fighter/rogue, but you also get swallow whole as class ability". I think MRPG works well enough somewhat ironically because for the most part the pregnancy aspect is part of the lore instead as making every class built around giving birth at the enemy or something (you have the broodmother for that, while every other class does its own thing).
If you did make a vore fetish RPG it'd probably be best to have it function on a point-buy system where you can build your own pred. Like you could put points into physical stats and unarmed fighting to abe able to subdue enemies in melee and eat them, or into abilities that let you incapacitate them in other ways to accomplish the same thing, or there could be an option to make the all-devouring voree-girl by putting all your points into abilities that let you swallow and digest anything but at the cost of the character being small and weak otherwise, requiring either an element of surprise or help of other party members to actually swallow any tough enemies.
...Still, I don't think it would work very well as a game. There's only so many ways you can make "I roll to eat the X" work as interesting and balance encounter design.

In a long running Shadowrun game one of the player characters got knocked up and was forced to do one more job while pregnant due to her being the only one with the skill set we needed for the run. The ST made it pretty interesting though since she did take a gunshot at one point but it hit her collarbone and so there was this neat contrast between her more womanly, human pregnancy and her upper torso sparking and smoking from her heavy cyberware system going on the fritz from the damage
But I find it harder to buy going on adventures while pregnant barring crazy circumstances
