That's not true, if you read the books the main character of that chapter always dies.
Not killing a character just because they're the main character is simply bad writing. Their must be a reason to their survival, and the lowest cop out will always be luck. But luck wasn't the factor here.

Cooper was shooting at Maximus while he was in the shop with Lucy for what felt like 5 mins. Time he could have used to switch to armor piercing rounds or think of a different strategy. Just feels like they didn't plan ahead and wrote in this vital flaw at the last minute.

he didn't have the perk yet to instakill power armor guys. he leveled up few times during the season.
Because he recognised him as not being a treat, and just messed around with him for shits and giggles. The Ghoul also used to wear power armour in pre-war days, so he can spot when a complete noob has stepped in one and is stumbling around like a fool. He instakilled the real BoS knights because otherwise he would be fricked.
To be fair, in the scene against several dudes in power armor the Ghoul held a different form of ammunition in his hand. Probably the only type of ammunition that can take advantage of the power armors weak-point. It looked like a drill.
Dude was using explosive ammo in the filly, and armour piercing in NCR headquarters.
