it gets easier as you level up and get more skills + gear, the first parts of Act 1 are some of the most difficult parts of the game depending on which objectives you go after, at least until you reach some of the more nutty parts near the end of the game

If you play evil you can be... pragmatic, selfish and actually get options to punish your enemies and make them wish they had never crossed you. If you are good you are expected to forgive, you are expected to help refugees just because they are refugees, you are expected to take war orphans that are an obvious trap into your camp.
Granted that if you are "evil" the games will always push for killing npcs and companions for no reason but you can ignore that kind of stuff and remain in character. That said i often prefer something more neutral and play the anti-hero Elric style. You know, helping the weak if it serves me and for a price. Killing anyone that looks at you funny is just a bonus.
