DLC/expansions should not add new areas completely annihilated from the base game, and instead new mechanics and content alongside the base game.
Like Binding of Isaac.
I have no interest in doing the DLC content that adds 4 hours of a micro-campaign since I already finished the main story. It just feels like filler and unimportant.

I only care if they are quantifal and quality as buying dlc means i probably enjoyed the base game alot. But new gameplay is very nice.
Souls games and nioh 2 have very fun dlc because they expand on builds ontop of added areas
If I have to go out of my way to reach these areas, like in Fallout New Vegas. I just don't feel like doing it at all, especially when I have to commit myself to completion until I can return to the main game.
Expansions are better if they are played during your playthrough rather than an aftermath, so my experiences were better because it just added to my playthrough instead of after i beat the game. So i guess expansions are best played if you havent experienced the game before

I've finished Dishonored campaign 3 times, and had a platinum on my PS3. Never touched any of the DLCs.

I think the exact opposite. Any DLC that expects me to play through the same content a second time with some differences only makes me feel like I got ripped of and wasted my time playing the "incomplete" version of the game. Same shit for early access for that matter.
It depends on the game. I mentioned Isaac for this reason, you can finish it in less than an hour, and is meant for repeated playthroughs.
A linear game would not benefit from an expansion that added more content to the base experience, but an open game would.
Expansions should add a comparable amount of content to the base game so possible a new campaign.
They should be both since they are asking you to pay again so the new crap better be worth it
