Tieflings are either so many generations removed from their demonic ancestor that their immediate relatives have no knowledge or expression of that heritage (If they're old school), or they're literally not even related to demons or devils at all, but are instead descended from a cursed empire (NuTieflings) and all of their ancestors are humans or humans cursed to be Tieflings.

IIRC technically being two generations removed from your demonic ancestry makes you a tiefling (the offspring of a human and a succubus/incubus is an alu-fiend/cambion, and the offspring of one of those and a human is a tiefling), though the majority of tieflings would be far more removed.

That's literally what tieflings are, at least originally. You had a whole table of random minor demonic traits your tiefling character could have, and while it was possible to end up with a red-skinned horned tiefling the odds were far higher that your character would look like a normal human but always smell of sulphur or have six fingers or something like that.

In my setting, you cannot start as a tiefling or a genasi. You have to become one by becoming infused with planar energy. This can be done with high level potions or rituals, or spending a lot of time on the corresponding plane.
