DS2 absolutely did not only take 2 years, they just had to cancel the open world and hastily scrape together a game from the existing assets.

Its not called a DLC but an expansion. They are also working on various projects obviously one releasing this year, DLC for the two year anniversary and a new game.
It wasn't planned from the beginning, they didn't expect the game to sell 20 mil units. They have other projects on currently as well.

My #1 pipedream hope for the DLC is for something like Chalice Dungeons to make a comeback, but expanded upon with a roguelike-style system when you start a new character and do a 'run' through it each time.
A linear, semi-unique, replayable PVE mode would have me playing ER for literal years to come.
