Either option makes you eligible for installing Tale of Two Wastelands (TTW) so long as you're not a moron and oust yourself for pirating it (opt A only) and/or don't ask about mod compatibility on the TTW forums (they get very ornery about muh proper modding wayz).
Hit up PCGW for patches for FNV, then install TTW. Ignore YUP as it'll be included in TTW.

You didn't change nothing, you didn't improve nothing, and it's more obvious when you play the DLCs after finishing the main story. All still the same.

An open world RPG is only as good as the contents of the open world.
New Vegas sacrifices some openness for a metric ton in depth for the main plots and the side quests.
Party members like Boone and Cassidy are still memorable, and Caesars just a way more interesting villain/faction leader than 3's Enclave.
In fact, that's the overall issue with 3. It's a retread of things 1&2 did better.
