There aren't nearly enough 40K games in general, but especially horror games. Imagine a game where you're a Guardsman or something getting chased through a hive city or space station by a Nightlords warband.

Just a shame that hes in the GW popcorn mines until further notice because corporate doesn't like getting dabbed on by a wildcard. To be fair, GW has designated "weird shit zones" like Ghoul Stars where eldritch shit happens. But this is a bit undermined by Warp being a thing and working on almost scientific principles.
That Rogue Trader RPG is meant to be set around the Ghoul Stars. Time will tell if they actually do anything with it or its just Chaos again with a fresh lick of paint.
The closest you got was the original Firewarrior during those last few Chaos sections. You learn to fear the audio cues of a Word Bearer loading in before he ackbars you with a missile launcher. The Chaos sorcerers were pretty spooky too since the only real sound they made was the chain lightning and teleportation. The oneshotting Chaos portals blocking the doorways were another thing to contend with. Despite how cheap the game was it did manage to set a tone when jumping between the various environments.
It's impossible to make a balance good enough to make the player character seem vulnerable, while also making the enemy threatening but not to the point where it should be installing the player from several miles away
Something like Darktide in scale would be needed but cutting out everything stronger than a mutant and no longer having the players be Emprahs Special Little Boys with titanium inch thick plot armor
