He doesn't go into the elevator at all. Trish is fated to lose her arm, the events that actually take place don't happen because the segment of time has been erased.
It's the same reason narancia can be impaled, it is fated to happen regardless of what actions diavolo actually takes in frozen time

That doesn't track logically because characters like Bruno don't move at all. If Trish was fated to lose her arm then you would also expect Bruno would be fated to have some kind of reaction to this. But instead what the story presents us is that Bruno's position is identical before and after the skip, as if though Trish's arm being severed is something happening outside of fate.

Isn't that backwards? GER erases the cause and leaves the effect. The effect of diavolo dying is permanent but the cause being erased makes him get stuck in a loop. The effect of his death is there but the cause is not so he is constantly dying in different ways forever.
You're misunderstanding what GER did to Diavolo. Him dying is the "cause" to trigger GER's activation to reset the experience to "zero". Diavolo doesn't get the "effect" of dying to resolve, which would release him from the loop, and is instead put into another situation where he would have to die since, as Araki decided to establish as a law at the beginning of Part 4, certain deaths cannot be avoided, such as Jotaro being predicted to die in Part 3, and finally having his death in Part 6; the universe is trying to force Diavolo to die, but GER is preventing it, which is why he is constantly in a state of being killed.

The way I was thinking of it would be to take a snapshot of the players inputs and then multioply it by the duration of time being skipped to force you to be were you are going to be in a few seconds when he does it
