I just use vanilla enhancing mods. Cut content restoration, more npcs traveling around the map, open Vegas strip, more animals/creatures across the wasteland, etc. I don’t like overhaul more like ENBs, gun mods, character overhauls, etc.
Viva New Vegas guide. Get basic bug fixes, utilities, and ui, and stop there. The rest of the list is optional.

For difficulty - play with JSawyer Ultimate mod. It rebalances the game, add additional things into the game, and reduces level bloat that is in vanilla game (you get 50% XP from quests and only 10% XP from kills) so you don't come out too OP. Level cap with certain things can be adjusted through ini file. If you're masochist you can turn on legendary difficulty mode, which basically works like Skyrim's legendary difficulty.
Additionally you can get Bleed mod which turns game into shooter game.
For content - New Vegas Bounties and MoreMojave.

I already have Jsawyer ultimate mode, but my problem is that I have already experienced most of the quests in the game, so I don't really bother with the quests that much outside of niche ones that I forget.
Though, the New Vegas bounties look interesting.
Uninstall.exe is my go to. Then you can play a non shit game instead.
