Except it wasn't. Not counting F76, which no one does, the first vault didn't open until like 50 years before Fallout started. Anyone left outside a vault would have died, mutated, or gotten sick and died. That first vault was purposely sabotaged like most vaults by Vault-tec. Then you had your mutated wildlife, ghouls, mutants, and eventually raiders to deal with. Lack of resources like food or water. Lack of building material that was structurally sound since you had no lumber or machinery to cut stone. And logically you also had the Enclave observing either sabotaging or kidnapping/killing people who might advance too far. People living in squalor makes sense. What doesn't make sense is how Fancy Lad Snack Cakes survived 200 years.
i always thought they should have explained the food by pointing out that nuclear war was a looming threat before the war, so supermarkets carried and advertised food which was capable of sitting inert on a shelf for a very long time, and maybe even resisting irradiation
