In every game, the Pokedex is stated be a high-tech device that automatically records. It has an AI inside of it that writes the entries itself. Or in the case of Gens 7, 8, and 9, a Rotom writing them.

They always use words like "reborn", and Ghost-type Pokemon don't consider themselves to be ghosts. In their minds, they're just as alive as any other Pokemon.
It's not in their mind. They aren't dead, they can reproduce, they have to eat, and they eat physical foods just like any other Pokémon, and can even die.
Ghost of Pokémon also have different properties than ghost Pokémon.

When Pokémon die, they don't turn into Ghost type Pokémon. They turn into ghosts of their former selves. Ghost types are simply species of living Pokémon with some supernatural/ghostly characteristics. They're not dead Pokémon even though some of them may have originated from the souls of dead humans according to myths.
