It's just a steep learning curve in general, I'd honestly just watch some people on youtube who are good at it and see if it looks like fun once you actually know what you're doing
most of the things you listed are features, they're designed to make the game unpredictable. the idea is to stumble blindly through it, time and again, finding random items and learning what they do until you work out how to tilt the scales in your favor, and what not to take under any circumstances.
Tips for newcomers:
>devil deals are a good way to get damage. beat the second level taking no red damage, get a power boost
>take cards, not pills. pill types are randomized across runs, cards arent, so you can actually learn something from the cards
>the end bosses unlock more stuff, beat them over and over with different characters until results happen

I played the game for 13 hours and still have no idea how to use cards or what they do. Also, I always just eat pills in every run because they can give you stat boosts. They also frick you over half of the time though. Is it better to just never take them at all?

Pills are, on average, a worse proposition than cards. more stat down and bad effect than stat up.
second point. if you can beat the second level without taking red heart damage, you get a deal with the devil that opens up. it allows you to trade red hearts for items, and the items it offers have a high proportion of damage ups

pills are total shit, it's hard to get any that actually do anything useful (balls of steel, stat boosts) because so many of them are just bad (stat drops) or useless (i'm drowsy, ex-lax, etc.)
by comparison nearly every card is useful in some way or another and it's way easier to break the game with them (speaking as someone who's only ever played afterbirth+)

if you clear the second level/floor without taking any red heart damage you're guaranteed to get a devil room, where you can get new items at the cost of reducing your max red health or 3 soul/black hearts if you don't have any red hearts at all
these can include damage boosts and other useful stuff
before you ask, you'll die if you spend all your red hearts in a devil room and don't have any soul/black hearts to fall back on or extra lives
you'll almost always find something useful there so long as you have enough health to actually get anything

i'll admit i'm not 100% sure what anon means by this because i am also not very good at this game but i will say beating runs with specific characters gets you guaranteed unlocks depending on what the last boss you fight is
if you're new to the game and have azrael unlocked i'd suggest picking him, he's a good character for brainlets since he starts off with a short ranged version of one of the best damage items in the game
just keep in mind he starts with 3 black hearts and no red ones, so taking devil deals will kill you unless you find an item that gives you a red heart

Not if you're just playing rebirth but with the dlc there's so many weird mechancs and items interactions that you often get a run where you feel like you can basically summon shit at will, like this

That's why I said to beat Mom, achievements are enabled if you've done that on your save file
You need to have Afterbirth+ or Repentence though. If you don't, AB+ kind of sucks on its own but Repentence is great and improves some of the AB+ content
ab+ was greatly fixed with the booster packs and The Forgotten though.
I love The Forgotten and do think the booster packs improve the game, but still can't recommend AB+ without Rep.
If you need one reason, remember that THIS exists on all floors in AB+ if you don't also have Rep.
I know I played a lot of AB+, even got all my lost hard marks, and had to deal with this shit every time, by now I already forgot about it because I started a new save for REP when the DLC released.
Still, the booster pack content is god-tier.

It's based on more on traditional Roguelikes than other similar games where this kind of randomness and obfuscation was standard practice. Those were the kinds of games that old, turbo-comp sci autismos would play while waiting hours for their code to compile on an old time sharing terminal computer. In any case some people play with mods that tell them what items do, but I think it's fun to embrace the chaos and try and remember what the different items are.
this game is designed as if it exists in pre-internet era where you would only play one game at a time for hours at a time.

NTA but CreamAPI is very safe. AFAIK nobody has ever gotten a VAC ban for using it, though I'm told some games have built-in detection for it. Isaac isn't one of them
