Don't think I ever heard about anyone struggling with the MGS1 torture. The one that people complain about is the MGS2 torture on european extreme, which got nerfed massively for the HD collection. The PAL version was really rough.
people "struggling" is just people failing it once
Some people just don't got the reflexes or fingers for it. And then you get Twin Snakes, where being TOO good at button mashing will have the game actually detect it as autofire and never end the torture so you die anyway.
Before you ask, MGS1 only checked for very specific controllers of the time, it can't do shit against emulators doing turbo-fire on what it thinks is a default controller
