I enjoy playing a medieval serial killer in Kingdom Come. I like to stab women while they sleep and hide their bodies. It's very satisfying due to the animated executions. I always try to do a quest that results in the most violence instead of whatever is right or wrong. However, being a bad guy is FORBIDDEN by the devs. It's absolute bullshit. Did you know that having a bad reputation makes npcs refuse to talk to you? So let's say a quest has two options, either you help the guards or you help the bandits. If you choose to help the bandits, you now can't do the other 10 quests in a village because your reputation is too low. Effectively, you either play a good guy or you can't play at all. The sequel needs to remove the reputation homosexualry. I like how the original Fable handled reputation. Either NPCs say nice things or mean things. It's that simple. The shit with shop prices, locking you out of quests, and stat bonuses ruin gameplay.
the issue is you dont get a bunch of quests as an evil character. assassinations, ruining competing businesses, stealing for people, a career of banditry etc should create an interesting evil playthrough.
Based After the quest of Johanka's trial, I killed that blacksmith's lying b***h with pleasure in the middle of the night, in her own bed. Sadly, the blacksmith awaken too, so he bites the dust too, not to mention their shop guard.

The closest thing with this much realism / sim I could think of is the forest. And that's a completely different game. I'll be happy to buy this one if it's even a 7/10 and runs stable on PC.
