It's actually funny because they did it to boost sales. Ultimate Spider-Man and the whole Ultimate line were dipping after Ultimatum (USM was around 50k) so they thought they'd do a shake up. Kill of Spider-Man and rake in the event bucks and news coverage, then replace him with a minority for more news coverage and more gimmick bucks. The only thing is that Miles sold even worse than Peter was selling beforehand once the shock factor went away. He was sitting just below 20k which is deep in the red for cancellation numbers before getting transplanted into the main comics because he's Bendis's pet character

It's probably something as simple as the face scanner being very close to the face and the Devs being too stupid to ask the modelers to fix it as they are scared of loosing muh realism. Women are more impacted as they have softer features you need to be careful with.

I also have a theory. They just have no idea how to apply makeup to a model makeing it look so much worse. They just slap some colours on and call it a day not knowing how makeup is actually applied. That's with some characters. I think it's different per studio but I think the main culprit is just not adjusting the model after scanning.
