do you rest your middle finger on middle mouse or right mouse button?
RMB. whenever i want to press down on the scroll wheel i move over the index finger, and for the extra button right below it i use the middle of the index.

fine moments aren't a problem if your setup isn't shit and prevents you from making vertical movement with your arm
if your posture or your desk height is shit
it will prevent you from aiming well in general

then i keep my middle and ring fingers together, ring finger is gripped onto the frame of the mouse right besides RMB, and my middle finger is resting on RMB.
my pinky is also gripping the right side of the frame like a claw but vertically and not horizontally like my thumb.
All 3, one mouse in each hand.

Get the microsoft intellimouse 3.0 if you dont like multibuttons, replaced the g400 for me since they are no longer made and its a great quality mouse

Finger tip by necessity. I have absurdly large hands. I have yet to find a mouse I can let it sit comfortably in a palm grip.
have you tried zowie ec1? it's probably the largest mouse you can get
