Worrying about it transforming into a super disease is like worrying about a jet engine randomly creating a black hole. There are some obviously morally-good choices they like, but for the most part the companions will complain no matter what you do during the faction quests. The key part here is that you choose a response that acknowledges their feelings. If you defend your decision, the companions get upset. If you just say something like, "I hadn't thought of that" or "Thank you for sharing your opinion. I'll consider that," then they don't get as angry.
