That's really what the story is about? I only had seen the translated synopsis for years (because this game is never ever getting tl'd) and I thought it was getting high marks because it was a heart breaker like Kana Imouto as it only describes the little sister having some kind of severe illness.
"Save her life with dick" would make my pp big, but I'd be so disappointed in every other metric.
I've personally always seen this get touted as a good e-girlcon game. Not that it has a good story. You have access to Maitetsu as an English-speaker. It's about the same thing.
Maitetsu is completely different isn't it? Monobeno's H scenes are part of the story, Maitetsu's are not and the game is just a train autist game with optional H content.

I find it hilarious that Maitetsu is allowed on steam because of this. Game still has a good amount of h-scenes with e-girls, but because they're optional side stories it's a-okay to have on steam. Meanwhile there's a bunch of VNs with very little h-content that still got denied because the h is a part of the plot.
The H scenes need a patch that you download outside of steam

It has been a while since I played, but from what I remember her disease is an inherited trait from her mother that makes her age rapidly, thanks to magic her father used the condition is halted and accelerated age is reversed if it occurs, but the strength of the magic is fading, in part due to her maturing and unstable emotions coming from her love for her brother. All routes eventually deal with the issue, either restoring the magic potency or using modern medicine to cure her. For the latter, keeping Natsuha mentally strong is required, so her brother, the one she loves, needs to frick her. At least I think it was something like that, I played it years ago.

It's 4x longer than steins gate and nobody would buy it if they do translate it. They tried with the base maitetsu and english sales were really bad

I read it a long time ago and knew almost immediately that this would never get translated. It's filled to the brim with obscure dialects, archaic language, references to obscure japanese folklore, and a bunch of other things that are just straight up very difficult to translate to English. I've done JP to EN translations before and just thinking about trying to translate this gives me a headache. I've heard of translation projects that tried and they gave up, not surprisingly
Same issue with Tasogare no Sinsemilla, amazing game with also a blood related imouto heroine true route, with a rural setting and a lot of Japanese folklore, even won best VN of the year. Will never get translated because it's too big and too difficult to translate.

As someone who can read VNs in Japanese, I think AI might be the best bet for people who don't have the time to study the language. Translators are a joke and there's a just a lot of good VNs that have 100% no way of ever being picked up.

How does that work exactly?
Compared to traditional machine translation, GPT-4 is much, much better, primarily because it can take advantage of context. So when prompting it with 200 lines of untranslated text, you can also include a few thousand tokens of preceding (translated) dialogue, as well as a lorebook/term glossary with brief character bios and such. This improves the accuracy substantially and helps it infer pronouns/subjects correctly when omitted in the original text.
However it's definitely not perfect, and while the output is way more coherent than MTL and the prose flows better, it's still kinda dry. It's also very expensive... I'd guess like $500-$800 of API credit to translate a VN of this size depending on how much of each prompt you allocated towards contextual information vs the text you actually want it to translate.
Would need some scripts to automate the process of iteratively querying the LLM and including relevant terminology, but I don't think that'd be that hard. It's the cost that is stopping me or I'd totally give it a shot for the novelty.
And then everything sexy or controverse gets automatically censored.

ChatGPT does, the API does not.
Ganker has been using GPT-4 and Claude to write explicit smut, including dicky, for a while. It's completely trivial to get around the reinforcement training, especially when you provide it with a context filled with (japanese text > english text) examples. The LLMs natural inclination to continue following the pattern (translating text) will override the very weak reinforcement training that tries to nudge it away from explicit content.
I'll probably check out the existing MTL, see how shitty it is, and then compare that against unedited output from GPT-4 to see how substantial the improvement is. The cost is not completely unmanageable, but if the (presumably edited) MTL is shitty-but-usable, it's not really worth spending $700 on an improved, but still inferior-to-human LLM translation.
Honestly it's not that pricey if there's enough interest in it.

Pretty much, just get a full clear save file and select the H scene from the menu. With the exception of 1 or 2 scenes, all of them exist outside of the story.
