Punishing but rewarding with plenty of "aha!" moments. No handholding. You're thrown into the world and have to figure out shit by yourself.
There's always the second Fear and Hunger game, Termina.
Been playing it recently and it's great, same idea with a much larger scope.
Otherwise, maybe Lisa the Painful? Much more comical in tone but features some heavy and depraved moments, and plenty of big choices where there's no right answer.
Termina is cool at first but very unfinished.
LISA is definitely the closest game to F&H and execution.
Other games that have a brutal learning process are the original Demon's Souls/Dark Souls (NOT anything From made after those) and stuff like Ninja Gaiden/SMT. I would also say Pathologic 2 on Intended difficulty is worth checking out.

I don't know how people tough through it. This game seems to be based on trial and error style of learning, but at the same time the rate of learning is hampered for me by how punishing making an error is. In the end I take no risks and avoid danger as much as possible, but this seems to leave me even more unprepared for later sections.
I don't know if it's the fact I insist on playing on the Fear & Hunger difficulty (which I've been told is the intended difficulty) or what. It's a shame, because I really love the vibe this game is going for, but it is punishing beyond my tolerance.

If the "difficulty" is based on rng and trial and error then I wouldn't even say it's a difficult game. I'd say it's a tedious time wasting game. Maybe you might find "fun" in blindly lucking your way through the game but I can't exactly imagine it's rewarding.

I found Termina really hard to get started in, Needles was a colossal roadblock at the beginning and the only equivalent to him in the first game, Crow Mauler, can just be avoided with the movement trick you've learned. But Needles will chase you down over and over.
It's extra bad because he's piss-easy once you know how to kill him but I was stuck there for hours until I just got depressed and looked up how to kill him. So yeah, the first game I think has a much easier learning curve
