You can then either load your gen 4 sav file onto an authentic cartridge using Checkpoint on a hacked 3DS or Godmode9i on a hacked DSi to transfer up to gen 5 using two DSes, or use an emulator that supports DS Download Play like MelonDS. Then for 5 into Bank you can either load it onto a real gen 5 cartridge, or use Dream Radar Cart Redirect to make PokeTransporter read your sav file instead of looking for a cartridge. Once it's in Bank you can move up to Home normally.
