UE5.1 has fixed Stutter a lot. You developers can also utilise PSO-caching to entirely eliminate all shader and texture/vfx streaming.
The only stutter you cannot fix is level loading/unloading.
Lords of the Fallen uses Nanite + Lumen. 38gb in size. The engine has nothing to do with file size. It's all about how developers compressed their game. Good developers will use good tech properly.

Fully nanite trees/wpo/Masked assets are too expensive and not performant. Nanite Landscape is ridiculously expensive. Still cheaper to use traditional landscapes.
The good engineers are working for Epic/Support workers for studios, or are hired by the big studios like Coalition.

Take CDPR for example, witcher 2 and 3 are good games and the devs had soul then they let W3 success get to their head, employed diversity hires, the old guys left, nobody knows how to use their engine and CP2077 is a mess.

Very low entry floor. Easy to learn, hard to master. And once you learn the basics, you can write it on your CV that you "know" Unreal even though you barely do.
Unironically Fortnite. Once they realised they need to make that thing work like a clock they've dumped insane amount of work into the engine, inventing new optimization techniques as they go. Unfortunately this means a lot of other experiments have fallen off, but then we're also getting a lot of cool new features, all driven by the zoomer royale. It's very weird, but I'm happy all of those talented people are now actually motivated to do good work.
