ER is a bit conflicted because it wants the story to be hidden in items like DS but also wants to do big cinematic boss moments and it's really awkward. This also ties into the whole "why can't I just talk with Malenia" issue.
This one always struck me too. Like "You're gonna think this is nuts but I totally cut open a clear path to your estranged brother."
They struck a good balance with Sekiro on this, we know of Genichiro and his goal, him coming back from the dead for his second phase is not as non sensical.

My personally theory is Placidusax was the Elden lord when the greater will and the frenzied flame were the same entity known as the One Great. When they split Placidusax was damaged and lost 3 of his 5 heads.

There wouldn't be any Erdtrees or Elden Lords if there is a One Great.

The Japanese translation implies that the Elden Ring/Beast is a parasitic entity with the word in in regard to Marika being its "Host".
The tree is implied to be a new entity that sprouted because of the Greater Will's influence or pre-dates it, there's no proper references for it.

I saw Hoarah Loux at a site of grace in Leyndell yesterday. I told him how cool it was to meet him in person, but I didn’t want to be a douche and bother him and ask him for remembrances or anything.
He said, “Oh, like you’re doing now?”
I was taken aback, and all I could say was “Huh?” but he kept cutting me off and going “huh? huh? huh?” and whirling his axe front of my face. I walked away and continued with my leveling, and I heard him chuckle as I walked off. When I came to pay for my runes I saw him trying to warp away to with like fifteen levels all in Strength without paying.
The maiden at the grace was very nice about it and professional, and was like “Sir, you need to pay for those levels.” At first he kept pretending to be tired and not hear her, but eventually turned back around.
When she took one of the levels and started marking it multiple times for Strength, he stopped her and told her to allot them each individually “to prevent any golden infetterence,” and then turned around and winked at me. I don’t even think that’s a word. After she checked each level and put them in the stats and started to say the runes count, he kept interrupting her by yawning really loudly.
Literally the best cutscene in the game. Wish hosts wouldn't skip it then watch the second one fully.

All of this seems like it could fit into a single year but I have seen people claim the shattering is a 1000 year war.
The actual ring being shattered could've happened 1000 years ago. The war after that and the aftermath could've also been for hundreds of years with it "ending" and the survivors just defending what they own, fighting over scraps and so on. I don't imagine Caelid immediately being covered in rot it slowly encroached and creatures and people ate the corpses and mutated over time.
There are wandering nobles that lived long enough to go insane from the passage of time so it was definitely more than a lifetime since the ring shattered.

I looked it up, apparently Gransax (the dragon in Leyndell) was killed BEFORE the war against the dragons, and his death kicked off the war in the first place, so his body has been there for even longer
It's a case where the art team and the level design team made a cool set piece and the story team poorly explained it.
That dragon is Gransax and the description of the weapon Bolt of Gransax says

Well before, there was an entire golden age created through Marika after she conquered the entire land in between. This involved her conquests and also technical Radogan's conquest since the Carian people were basically fedora tippers until Rado and Rennela created the Golden Order ideology (Greater Will's Dogma mixed with scientific observation by Carian sorcerers).

Honestly I feel like I've missed half the game despite playing 75 hrs, I kinda lost the will to continue after the 10th ball bearing reminding me of another missed questline.
I guess the point was to look up a guide uh.
The game was unironically made with Ranni's quest in mind but I guess they wanted to justify the other endings. Probably should have forced you to do the quest and then included a branching point where you can choose to help her further.
They kind of did that, but not really.
The issue is that you don't encounter other characters organically like in DS, for example no one missed Solaire or Siegward or Anri.
In Elden Ring you have to go and look for other characters or else you'll miss 60% of the cast and potential content.

The issue was making the world too big for their usual methodologically of quest design. It worked when it was tight, interconnected areas but now everything is 100x more spacious and NPC's will move to incredibly nonsensical areas. Why does Patches go to Shaded Castle after you uproot everyone from Volcano Manor? If I had cleared out that place before defeating Rykard what reason would I have to go back to check if an NPC was there? I discovered Blaidd being in the Evergaol on accident by just randomly deciding to walk around Limgrave otherwise I would've likely missed him. If I roamed around Liurnia before Weeping Peninsula I would hardly encounter Hyetta and her main locations don't make any sense to begin with.
at least patches and blaidd are completely inconsequential at that point in their quests

It's implied that some culture on the surface, probably Carians, attempted to replicate the silver tear but failed, creating the Albanaurics.
I think, or Albanaurics are offshots of silver tear. The problem is the Silver Tear can become true beings as shown by the cut Asumi quest.

Godfrey's dialogue has been cut up extensively. He used to be an NPC that sent the player into the Lands Between to follow the guidance of grace.
