I blame Carmack, he's one of those moron autists who literally just work 12 hours of the day, 7 days a week and don't understand when others don't want to devote their whole life to their job. This is also why iD's games got completely soulless once Romero left, he was the soul of the company, Carmack was the brain.

He had some seriously creatively talented people and he did them wrong
Despite the lore that Carmack was an unstoppable technical genius, he actually started having repeated issues working through Quake. He bit off more than he could chew.
He was regularly making mistakes and doing engine rewrites that forced the rest of the team to rewrite their tools and rework levels.
Romero was happy to chill and give Carmack the time he needed, but Carmack became obsessed with sticking to a fixed delivery date. Which is ironic because Carmack was the one struggling.
Also, quake was the first time that Carmack wasn't the smartest person in the room, as he had hired Michael Abrash from Microsoft to work on quake, who was a legendary low level programmer. Carmack probably wasn't super comfortable with this.
Basically Carmack had a minor mental breakdown because he had to face the fact that he wasn't actually an unstoppable programming wizard and projected his insecurity onto the rest of the team.

Carmack kind of touched on this with his recent podcast with Lex Fridman.
Tldr, Carmack with his age more or less said "I wish I could go back in time and not be such a prude", but also still alluded to the fact that he was justified in feeling annoyed by the fact he was doing a lot more work than Romero.
I do agree that Carmack was the brain and Romero was the soul. It's just unfortunate circumstance. I hope these two autists can work together again one day.
They were both at fault. Carmack is a genius with the personality of a hallway monitor, and Romero is a creative visionary that let celebrity get to his head and thought he was unstoppable, which made him unfocused and uncaring.
