The game has a super clean environment compared to DS1/3. Minimal skeletons if the theme of the area isn't "Skeletons Everywhere". Minimal depictions of gore. It kinda seems like they were going minimal on the dark part of Dark Fantasy.

I think it has more direct gore than DS1, but the main difference is it isn’t as bloody and the grotesque & sexual stuff isn’t highlighted as much. The difference between that rock lady having big knockers and Qualaag having them is that the game put much more emphasis towards those breasts on her essentially naked body. It’s still rated PEGI 16 over there, though. And the ESRB barely means anything on a year by year basis, like I think some Looney Tunes game had a T rating a decade prior.
I’m pretty sure people have done In depth analysis on the game that shows that it does have a strong connection to trans sexuality and it supports the general notion of it
