Because aiming is the most important thing with a mouse, hitting it on the side is detrimental to that even if only slightly.

Tried one of these, but I don't play games that would actually take advantage of the extra keys, so I ditched it. All the extra keys did was get in the way.
How do you reposition your mouse without moving the cursor with these? You can't lift it up by gripping it with your thumb and ring finger/pinky without pressing buttons
MMOgays don't need to move the mouse very much. They just click on a few menu options and then use the side buttons for their cooldowns.
ive had an original Razer naga for over 10 years now

The lower one, depends on the game. In FPS I bind them to alternative scope/sights, while in TPS I bind it to switch camera shoulder.

The little button is always interact. Very handy in shooters so that I never have to take my fingers off the movement keys. The big button varies, I use it for melee in Destiny and transference in Warframe.

Depends on the game of course. When I was only playing Titanfall 2 I had them with tactical + grenade and that was very useful for playing as titans too (usually shield and special attack). I could do those easily while moving. Some people liked to use them for crouching too.
I quickly noticed that nobody would use their shield ability while walking to the left because they would have to press q, wouldn't use the attack while walking right because pressing g. It was weird to win so many fights because of stupid reasons like that.

i literally fan the the crouch button with my the middle section of my thumb while i jump i dont toggle at all
but weve established that you have hooves for hands so if its that much trouble you can swap them out

In games where the default button for scope, melee, grenade, etc are a keyboard button by default, I usually map those to it. In FF14, I map 3 of my side buttons to alt, ctrl, and shift modifiers. Else, push to talk
I have the 12 button side, but their uses can be for any number of actions or menu function. I tend to only use the 6 closest buttons though.

Left/Right Lean if the game has it. Otherwise nothing. I usually see it bound to voice chat but I don't like adjusting my aim grip to hit a button, VC is better bound to a key (or even better, open mic)

Pinging. I used to use M3 for grenade, but several games have mouse wheel to switch weapons without the ability to toggle that dogshit off.
Nothing, they're too far back on the mouse for my thumb to naturally rest on them, so it's not worth trying to remember to bind something there.

For fast-paced shooters, magnify optic and reload.
For slower paced shooters, usually something more specific. For example, for Escape from Tarkov, I have them set to Page-Up and Page-Down because those are the optic magnification buttons when in a match, and when I'm in a menu, they snap to the bottom or top of any given list.
