The conspiracy unfolds and the twist is that the asylum part is future and flashbacks are actual present time and his ultimate psychic power was active from very beginning which was ability to see the future
Not sure if its the greatest, but I was so invested in Xenogears I didn't realize that ID is an existing member of the cast. On one level it seems quite simple, but on another it was done so well for its time that you had other rationale to look around their absence, that it was one of the best. The game had a number of other twists too, but its a pity that we didn't get the Disc 2 that was originally planned Ramsus NOT being recruitable if you did the right things was such a kick in the dick that when I learned it was originally planned, it made perfect sense that it was planned but cut and just Citan's real identity, weapon and other shit is enough on its own when finally revealed
Bioshock didn't age that well overall imo but it's hard to think of a twist that's more iconic because it isn't just about the game's story but the nature of gamedesign itself.

Also, his environment changes to fit the nu era, he makes sure to talk a lot and be talked to, especially about the things he's about to do.

The one that impacted me the most was in this series of flash games called Submachine. Throughout the series, you see no other people, and the extent of any presence of people are notes left behind across the different structures by a man named Mur and later some by a woman named Liz
Near the end of the series, you eventually reach Mur and Liz's tomb, along with a note seeming to depict their last moments. Then, in the final entry, the (rather well foreshadowed) reveal that the "light" Mur had been experimenting with is time, that the Submachine itself is in the future, and that not only are Mur and Liz alive, but they went on a date in their own tomb. The last scene in the series is escaping the machine, and, at last, seeing the two of them.

Not the 'greatest' but one that caught me genuinely off guard was the fakeout ending of Ni no Kuni, the cutsey Studio Ghibli JRPG.
You beat the big bad villain who's been built up the whole game, satisfying Ghibli appropriate ending where the power of love heals everyone, the game's already like 50-60 hours and you've explored 90% of the map so the last area must just be post game right? and then it turns into a fricking horror game where all your cute cartoon friends turn into genuinely scary feral zombified monsters and you've got another 20 hours to go playing through the end of the fricking world. I wasn't even a kid when I played this game, I was a grown ass adult and that fricking tone shift was so out of nowhere I was legitimately scared.
I always liked this one because I've never liked being "the chosen one" in games. Of course, this was much more impactful in 2002 before Nomura gave everyone a keybalde.
See, I feel like this one never made any sense, especially for how short the whole sequence is. It feels kinda pointless and it didn't do anything really strong narratively besides the twist itself. It didn't really change the status quo except for like two screens, and giving you an OP companion that can bulldoze enemies zeros out any of the emotional weight you're suppose to be feeling in that moment; lost, abandoned, questioning everything that happened so far. It could have been utilized better - but then all that lore got thrown out anyway, so it's moot I suppose.
