this is explained in silent hill 1, they're basically psychic projections, except there's the implication in 2 that since none of them are young abused girls like alessa their psychic power is so weak that it only affects them. the town is like a psychic ability amp, and the closer you are to being an abused little girl the more power you have (an old trope). this is also why angela can project a little of her world over top of james' because she was abused as a little girl, but since she's an adult now she's weaker. i don't think it makes PRACTICAL sense but it fits into the cliches and if you put this metric to everything in the original games it fits, the only anomaly is walter being able to do multiple-people psychic projection despite being an adult male outside of silent hill, but 4 is the odd one out in most other ways too.
i should add that a little girl who hasn't been abused, such as laura, has no psychic power. there's this fictional trope of sufferers of great trauma opening a third eye that silent hill seems to really like, but it also really likes the abused young lady thing between alessa/heather, claudia being key figures and angela being the "strongest" psycho-hallucinator in 2
