If I recall right, the reason for him losing an eye was that his Zweillous/Hydreigon attacked him after being mistreated and undergoing very intensive training to make it evolve earlier.
This was always my headcanon, I don’t know if it was confirmed anywhere though.
This hasn't been stated anywhere, but it's pretty reasonable given how vicious Hydreigon is.

Looks cool as frick but whats with all the horizontal eyes and shit? Does it have any meaning? There's an odd focus on eyes with him in general, so it doesn't seem like its meaningless. But who knows? I love that kinda stuff.
It’s not confirmed, but it is my headcanon. His Hydreigon knows Frustration and actually has minimum friendship (which is something no other NPC’s Pokemon has), which proves that it does hate Ghetsis. It’s reasonable to assume it hates him enough to maul him and cause his damaged eye, arm, and lip.
There's also the fact he has an underleveled Hydreigon meaning he likely forced it to evolve early somehow. There is also the fact that Hydreigon is known for being incredibly violent and attacking anything that moves. It's probably the most widely agreed upon theory that his injuries are a result of his Hydreigon.

He does have an arm, but it may be seriously damaged and unusable. In his BW2 artwork you can clearly see it bulging through the coat. His Masters model also has both hands but there's no visible skin, and according to bulbapedia he holds his right arm behind his back.
In BW intro you can see him using both hands, but the right is dark. No idea if that's a glove or some burn/scar; either way I assume N's coronation happened some time before the start of the storyline and Ghetsis' hand became useless after that, either by some accident/attack or progressive damage.
A functional eye and a missing one. A disabled half and normal half of his body. His BW1 robe has different colors on each side. In the screenshot with the crown he literally has A WHITE HAND AND A BLACK HAND. Also, his two-faced nature. Ghetsis was literally designed to be a walking yin/yang symbolism.

so I am breeding at the moment for the PWT in BW2 how OCD do I have to be in order to be able to clear everything when it comes to the IVs or can I just focus solely on proper nature and EVs?
If I recall, PWT trainers have an IV floor of 20 for all of their mons and are EV-trained, so having good breeding can help, but won't break you.

Most agree Ghetsis lost his eye due to some kind of "training accident" with his Hydreigon (which explains why it knows Frustration, a move for Pokemon who REALLY fricking hate their trainers)
His lip and right arm are probably from the same incident, but I also read that it came from him getting too close to Kyurem and that fricked up half his body.
Also anyone think it's really cool that his "solid eye" is white in BW1 because he claimes to hold absolute truth but then it's black in BW2 because it's clear he's fighting for his own ideals? B&W has such amazing lore tbh
